  • In case of emergency (urgent/dynamic situation – especially if in result of a crime you are injured, you need immediate medical assistance, your life or health is at risk): call 112 and seek for assistance (available in English).
  • In other cases (if you were affected by a crime but your case is not an emergency, you don’t need immediate assistance of the Police, ambulance, fire department etc.) please fulfill and submit the following form (be aware that you have to be logged in with your @student.uj.edu.pl, @doctoral.uj.edu.pl or @uj.edu.pl e-mail account):
